If you provide instructed/guided/led activity sessions or courses, please complete the section below.

For each of the activities you offer (from the drop down menus), please indicate who your most experienced and best qualified 'specialist' is. This person (called a technical adviser by the Licensing Authority) may be you, or an employee, or a regular contracted freelance, or some other external activity specialist. This person should be able to provide advice of a technical nature relating to the activity, should you need it. They are also the person who has the experience and qualification to verify the competence of any unqualified staff you may use. Give this person's highest, relevant qualifcation in each case, or say 'none' if they have experience only.

In the 'Notes' section on each activity, please jot down any 'sub-sports' that you offer under the general activity heading (eg under Motor Sports, you may add 'Quad-bikes and motorcycles', or under 'Sailing/Windsurfing', you might say 'sailing only', or 'cruising yachts only' etc.) Also in 'Notes' you could give an indicate of the upper limit at which you operate and an example of a typical venue (eg Kayaking up to Grade III rivers, or Orienteering in woodland such as Grizedale, or Archery 'on site' only, etc)

This form has space for up to EIGHT different activity groups! We don't expect that anyone will have to fill them all in - in fact for most people just one or two plus some notes will suffice!
